A pregnancy is always accompanied by physical changes, various medical considerations and plenty of questions for patients, especially if they are first-time mothers. Having an obstetrician involved from the early stages of your pregnancy can help you find a sense of clarity, giving you a consistent anchor to turn to as you navigate the uncertainties and challenges that pregnancy brings.

Understanding what to expect from your obstetrician throughout your pregnancy can also help you feel more prepared for the months ahead, from your first appointment and routine check-ups to the tests, scans and medical guidance you may receive along the way.

What Is Antenatal Care and When Does It Begin?

Antenatal care refers to the medical care you receive during pregnancy to monitor your health and your baby’s development. It consists of regular appointments with an obstetrician, along with appropriate examinations, screening tests and ultrasound scans at different stages of pregnancy.

Antenatal care usually begins after you find out that you are pregnant. Your first appointment is often scheduled at around 6–8 weeks of pregnancy, although you may be advised to see an obstetrician earlier if you have pain, bleeding or other potential complications.

Starting antenatal care early allows your obstetrician to establish important information about your pregnancy and identify factors that could affect how you are monitored over the coming months.

What to Expect During Your Antenatal Appointments

The topics discussed during your appointments will change as your pregnancy progresses. Early visits will focus on establishing your overall health status, while later sessions will likely be focused on monitoring your baby’s development and preparing you for delivery.

Your First Antenatal Appointment

Your first appointment gives your obstetrician an overview of your health and pregnancy. You will usually be asked about your medical history, previous pregnancies, current medications and any relevant family history. Your obstetrician may then carry out a preliminary antenatal screening.

An ultrasound scan may be performed to confirm that the pregnancy is developing within the uterus, determine the number of babies and assess the pregnancy’s gestational age. This information helps your obstetrician estimate your expected due date.

Other assessments may also include measurements such as your blood pressure, height and weight, together with blood and urine tests where appropriate. Your obstetrician can also discuss pregnancy supplements, medications and any lifestyle considerations relevant to the early stages of pregnancy.

Routine Check-Ups Throughout Pregnancy

After your initial assessment, regular antenatal appointments allow your obstetrician to track changes in both your health and your baby’s development over time, as well as carry out risk assessment.

Depending on the stage of pregnancy, these visits may involve checking your blood pressure and weight, assessing your baby’s growth and heartbeat and reviewing the results of scans or screening tests. Your obstetrician will also ask about new symptoms or concerns that have developed since your previous appointment.

Preparing for the Final Stages of Pregnancy

As you approach the later stages of pregnancy, your antenatal appointments will increasingly focus on preparing for the weeks ahead. Your obstetrician will continue to monitor your health and your baby’s growth and position while discussing what you can expect as you near your estimated due date.

This is also an opportunity to discuss your birth preferences and delivery plan, including the planned mode of delivery, pain-relief options and any individual considerations that could affect childbirth. Your obstetrician can address your questions and help you understand the available options so that you can make informed decisions ahead of delivery.

Tests and Scans Your Obstetrician May Recommend

These tests help your obstetrician gather vital information across different stages of your pregnancy, also allowing them to screen for potential concerns that may otherwise go unnoticed.

Routine Blood and Urine Tests

Blood tests may be used to check for anaemia, determine your blood group and screen for certain infections that could affect your pregnancy.

Urine tests can identify changes that require attention. For example, glucose may indicate high blood sugar, while protein in the urine can be associated with pregnancy complications involving high blood pressure. These findings help your obstetrician determine whether further testing or closer monitoring is needed.

Ultrasound Scans and Foetal Assessments

Ultrasound scans allow your obstetrician to assess your baby’s development at different stages of pregnancy. Early scans can confirm the pregnancy’s location and gestational age, as well as whether you are carrying one baby or multiples.

A detailed scan during the second trimester examines your baby’s developing structures, including the brain, heart, spine and limbs. Later scans may assess your baby’s growth, amniotic fluid levels and the position of the placenta.

Screening for Pregnancy-Related Conditions

Screening tests may be recommended to assess your baby’s likelihood of certain chromosomal conditions or identify health conditions that develop during pregnancy.

For example, non-invasive prenatal testing (NIPT) uses a maternal blood sample to screen for chromosomal conditions such as Down syndrome. You may also undergo an oral glucose tolerance test to screen for gestational diabetes. If gestational diabetes is diagnosed, your obstetrician can discuss appropriate monitoring and management during pregnancy.

Screening does not necessarily provide a diagnosis. If a result indicates an increased likelihood of a condition, your obstetrician can discuss whether further testing or monitoring is needed.

Beyond routine monitoring, your obstetrician can help you manage the physical changes of pregnancy and address health concerns that arise along the way.

Managing Common Pregnancy Symptoms

Symptoms such as nausea, acid reflux, fatigue and pelvic discomfort are common during pregnancy. Your obstetrician can recommend suitable lifestyle changes or medications to relieve these symptoms, particularly when they begin to affect your daily activities. They can also assess symptoms that are unusually severe or persistent to determine whether further investigation is necessary.

Identifying and Managing Pregnancy Complications

Certain complications can develop as pregnancy progresses. These include gestational diabetes, where blood sugar levels become too high, and pre-eclampsia, a condition involving high blood pressure that can affect both mother and baby.

Your obstetrician may recommend closer monitoring, additional tests or treatment if a complication is detected.

Advice on Lifestyle, Nutrition and Medications

Your obstetrician can also advise you on everyday choices that may affect your pregnancy, including diet, exercise, supplements and medication use. This may include recommending appropriate prenatal supplements, discussing foods or activities to avoid and reviewing whether your existing medications are suitable during pregnancy.

How Often Should You Contact Your Obstetrician?

The frequency of antenatal appointments varies depending on the stage of pregnancy, your health and whether any concerns have been identified. Appointments may be spaced further apart earlier in pregnancy and become more frequent as your due date approaches. Your obstetrician will recommend a schedule based on your individual needs.

However, you should not wait until your next scheduled appointment if you develop concerning symptoms. Contact your obstetrician promptly if you experience vaginal bleeding, severe or persistent abdominal pain, fluid leaking from the vagina, severe headaches or visual disturbances. If you are unsure whether a new symptom requires medical attention, contact your obstetrician or seek medical advice for guidance.
